    What version of the Ultrabans are you using?

    What version of the Craftbukkit are u using?
    Latest 1.6.1 r0.1 dev build

    Please provide any additional information below.

    Could you add a option in the config to reset Max warnings like
    "ClearWarnings: 1d"
    so every 1 day it would do MySQL checks to delete all warns from the database

    because once max warns stack up, every warn just "kicks" this might be a bug im not sure
