I have a system on my server that promotes a user via PermissionsEx up a ladder for reaching certain milestones in gameplay. They get promoted up this ladder for 1 day of gameplay and then again for 100 hours of gameplay. However, I've noticed that some users, when they get promoted, lose their rank entirely. Here's an except from my rewards.yml. When they reach a milestone, a message is displayed and then they're promoted.

I don't know if this has anything to do with MYSQL errors, but I'll leave two reports that I've found recently when looking through the log:
