What Version of OnTime and Bukkit are you using?
OnTime: 3.13.3
Bukkit: 1.7.9 R02

What storage option are you using? (YML or MYSQL) ?

What permissions plugin are you using? (Groupmanager, PEX,bPermissions, etc.)

What steps will reproduce the problem?
1. Add an OnTime Reward for Rank Promotion after specific time
2. After required time promotion, rank up
3. Prefix stuck.

What is the expected output? What do you see instead?
The rank system works but the prefix is stuck which only occurs if I set rewards with ontime

Do you have an console log of what happened?
No.

Did you turn on OnTime logging (/ontime logfile enable 1)?  If so, provide a link to that file too.

For now, it seems even after I've removed the OnTime plugin, my prefixes are still stuck.
If I get promoted to the Member rank for playing 1 hour (requirements for Member rank), and I /manuadd my self back to Owner, I get promoted to Owner but my prefix stays the same.

Is there any way I can fix this?

P.S It seems the only way to fix this is to not use OnTime or any promotion plugin but if you guys know something about it please help!

I had another user with the same problem, he resolved this by upgrading to the latest version of GroupManager: His post:

I updated my GroupManager to v.2.1.27 from the pervious release. That seemed to fix the issue. Thank you for your time and I'll respond if I encounter anything else with this matter.
