Describe to us the issue that you're having:
Everytime I reload the server it adds groups moderator and guest.  I don't want these groups.  Also, when I do /promote (name) it doesn't follow the ladder I created, but instead goes from guest to moderator, and then to admin. Here is a copy of my world groups file http://pastebin.com/Pw013XTB

Which steps will reproduce this problem?:
1) change names of groups and add other groups
2) start/reload the server
3) /promote (name)
....

Do you have an error log of what happened? No error log what so ever

Which version of bPermissions are you using?: (Please don't say "latest"!!!) 1.4.2-R0.2

Which version of CraftBukkit?: (Please don't say "latest"!!!) craftbukkit-1.4.5-R0.2    beta build

Which OS? Windows 7 (though I will be transfering the file to my ubuntu server once it starts working)

Any other information we may need (plugins, configs, etc): permissions for the default group works great
