Wow, EXACT same thing happens on my server. Glad to know it's not only me experiencing this issue!

Usually what causes it is if a player relogs, but that's only what seems to be the cause.

    List of plugins:

    • bPermissions
    • WorldGaurd
    • WorldEdit
    • VoxelUpdate
    • VoxelSniper
    • Vault
    • TownyChat
    • Towny
    • SuperpermsBridge
    • Register
    • NoCheat
    • Multiverse-Portals
    • Multiverse-NetherPortals
    • Multiverse-Core
    • mcMMO
    • LWC
    • Jobs
    • iConomy
    • Essentials
    • CoreProtect
    • CommandBook
    • CommandBin
    • Citizens
    • BKCommonLib
    • NarrowtuxLib

Usually the problem is noticeable through chat. TownyChat has chat permissions to speak in the channel "global", and when the permissions break for a player, his chat is marked with no channel, so he can't hear anyone, but we can hear him. Usually multiple server reloads fix the problem, but it's still a nuisance :P
