Describe to us the issue that you're having:
When a player joins the server, it appears that a blank group (with no permissions and no inherited groups) is made with the name of the player who has joined.

Which steps will reproduce this problem?:
1) Have a player join the game.
2) Check the groups.yml file - a group has been made with the players name.
....

Do you have an error log of what happened?
There aren't any error logs per say, but it does appear from the debug message that each event happens twice - ie the "[debug] Setup superperms for xyz. took x ms." get logged twice.

bPermissions v2.10.4

Which version of CraftBukkit?: (Please don't say "latest"!!!)
Spigot b841 - same effect with CB

Which OS?
Ubuntu 12.04 LTS (x64)
